For applicant having graduating from a recognised Clinical Physiology
Degree Course...
From Jan 2007 a new form was made available for
graduates of the BSc Hons courses in Clinical
Physiology and Audiology.
After consultation with the Health Professions Council, and in recognition that
these are the recognised entry qualifications; the forms and evidence needed has
been modified appropriately.
Only students outputting from a BSc (Hons)
Clinical Physiology or Audiology degree from an accredited HEI as shown on the
Accredited Provider List will be
eligible to use this form for application to the register.
Please note that when
using this shortened version of the form that a reference is required This must
be a character reference from your workplace, wherever possible but if this is
not available, from your work based assessor/clinical tutor/internal verifier.
The character reference should provide evidence the applicant's fitness to
practice.

must be made on a recognised RCCP form which is available in 3 formats.
Paper
You can obtain a paper copy of the Registration Form by sending a large A4 stamped, addressed envelope to the RCCP Administrator.

PDF (Portable Document Format)
You may download and print a blank form, to be completed by hand, by clicking on this link - Application form as a PDF.

eForm
You may download the eForm by clicking on this link - Application eForm.
Once the eForm is saved on your disk drive you can fill it in on screen; the data that you enter is saved as you work and you may return to it as many times as you wish.
If you require more sheets of certain pages either download the eForm again (taking care to save it as another filename or you will overwrite the data you have already entered) or download the PDF version and print off extra copies of the pages that you require to complete by hand.
Please note that you must print out the eForm for submission by post.
Please do not distribute the eForm via email this is a contravention of the software license. It should only be downloaded directly from this website.

Applications can be written or typed. Handwritten forms should be completed in black ink.
Your proposers should be members already on the register who can vouch for your practice. You can view the current register on this website.
When you have completed your ‘Application for Registration’ form submit the original plus one copy with a cheque for the registration fee. See the latest guidance notes for details of the fees . (Please note that this is an annual fee due for renewal on 1st May each year.)
If incomplete forms are submitted a further registration fee may be payable to cover the extra administrative costs of processing the application.
Submissions by Email will not be accepted (and will also contravene the software license for the eForm).
It is recommended that you keep a copy of the form for your own records and in case it gets lost in the post.
Post your completed application to the RCCP Administrator.

In response to many requests for
the introduction of Direct Debit payment for renewal fees, we are
delighted to announce that this will be the mandatory payment method for this
year and subsequent renewals. Please complete and return the
Direct
Debit Mandate Agreement as soon as possible together with your
signed
Renewal Form and Declaration and post to the
RCCP Administrator.
Without both these documents we will be unable to process your renewal and issue
